This form is to be completed for PROJECTS only. 

Equipment may be ordered without knowing user information.

The responsibility remains with the Project Manager/Coordinator to complete the New User Form (found on the Aspen Medical Portal under e-Forms), when users are known, to create the associated user accounts where appropriate.
